Your First Visit
No matter how many times you’re seen we’ll want to know everything that’s going on with you, but your first visit requires some specific attention.
Your initial visit will last approximately one hour and here’s how it will go:
First, make sure you’ve brought shorts, a tank top, your insurance card, a photo ID, and any imaging reports (X-ray, MRI, etc) along with you.
Then, you’ll meet with one of our staff members to fill out your paperwork and go over your medical history.
Next, You’ll meet with Dr. Zak for your consultation.
After the consultation a functional exam will be performed to assess movement patterns.
Then some neurological and orthopedic testing will be conducted.
Dr. Zak will then discuss your diagnosis and propose a treatment plan and prognosis.
Treatment will be tailored to address your specific issues and to realize the best possible outcome in the shortest time possible.
Payment is due at time of service. Accepted forms of payment are VI, MC, Check, and Cash. We Accept Health Savings, Health Reimbursement, and Flexible Spending account payments.
